We at STRIVE4You value your feedback! As part of our Grievance and Customer service policies we have developed this form to give our clients, participants, volunteers, partners, and staff the opportunity to ensure their voices are heard. We ask individuals address their concerns with the STRIVE4You representative in charge or their superior when possible. However, this form is for those instances where an individual would like to share information directly with the Board of Directors and may want to submit their concerns anonymously.

This form is set to send an email directly to the entire Board of Directors all at once upon submission. We hope this will give you confidence to submit your concerns without fear of retribution.

STRIVE4You encourages employees, volunteers, and others related to the organization to report any violation of policy, procedure, or ethics, illegal activity, or other misconduct by employees, volunteers, or others related to the organization. No person who in good faith reports a violation shall suffer harassment, retaliation, or adverse employment consequences. An employee, board member, or volunteer who retaliates against someone who has reported a violation in good faith is subject to discipline up to and including immediate dismissal/termination of employment as part of policies outlined in our STRIVE4You Code of Conduct.

In the below form you will notice that fields for your name, email, and phone number are optional. Only provide this information if you would like a member of the Board of Directions to contact you for follow-up questions or to update you on the situation as handled. The Board of Directors will initiate an appropriate investigation to all complaints in a confidential manner whenever possible and are happy to provide you with periodic updates if you provide such information to do so.
